The brand and model of the bidding performance product should be consistent with the brand and model of the bidding product.（2）投標(biāo)人須按規(guī)定格式提交業(yè)績(jī)表，并提交相關(guān)業(yè)績(jī)證明文件。業(yè)績(jī)證明文件至少包括：1）銷售合同復(fù)印件（內(nèi)容至少包括：合同首頁(yè)、合同簽署頁(yè)（應(yīng)有雙方蓋章）、合同簽署時(shí)間、制造商名稱、貨物名稱、業(yè)績(jī)產(chǎn)品品牌及型號(hào)）；2）用戶簽字或蓋章的到貨驗(yàn)收材料復(fù)印件（到貨驗(yàn)收單或調(diào)試驗(yàn)收?qǐng)?bào)告或使用方出具的有效證明文件）。合同和到貨驗(yàn)收材料原件備查。(2) Bidders are required to submit performance forms in the prescribed format and relevant performance certification documents.
